NHS Failings

 

Mrs Jackie Wright was discharged from St. Peters Hospital on the 6th November 2006 to the Norfolk House nursing home without the next-of-kin being informed. She was discharged without any clinical notes, she had no medication, there was no proper handover and an open wound on her leg that I had previously been queried, was not dressed and  weeping. She was deposited on the Nursing Home without any prior warning. The next day at 10am she was dead. Take a look at the photographs. Should anyone be discharged in this way and what does this say about the intelligence of the Nursing Staff and Doctors at St. Peters Hospital?

A facsimile was sent by me the very morning she died personally addressed to the CEO, Mr. Glenn Douglas followed by a letter. On the 8th December I receive a 'Report' which in the summary reads; 'I acknowledge that the omission of the relevant paperwork, medications and dressings was not to the usual high standard of care offered to patients and I sincerely apologise for the inconvenience ..,.'  All in all, a load of whitewash. Again it was impossible to contact Mr. Glenn Douglas, but we immediately reported to Joanne Edwards, who holds the title, Assistant Director - Acute Services , the investigation that had been promised was erroneous, incomplete and omitted:

  1. Why there was no official handover.
  2. Why her hygiene was so poor.
  3. Why old dressings were left on her upper arm and lower limb
  4. Why no medication was passed over.
  5. How the hospital could possibly discharge her with open wounds as seen in the photographs already in their receipt.
  6. Why  letters  marked Private & Confidential and addressed to a specific person, that person bearing the title as CEO, does not have the courtesy or the guts to actually pick up the telephone and speak to me when he has been specifically asked to do so.
  7. How many old people are being discharged in this way.
  8. The poor standard of nursing contributed to her death and I don’t consider this is adequately reported on the Death Certificate and effectively is done to fudge the figures.

I was promised she would look into the matter and hold a full enquiry. Since then, nearly two months later, we still await their findings..
